Initially we were just going to punt on the bicuspid, but it was about 30 seconds to get the length and shape…so I just did it as I had the molar isolated.  Symptoms resolved, and with this fat root that is longer than it looks, I thought of Tom Cruise in Risky Business.  😀

The patient was a super easy-going young guy, and was game to give it a try. 

(Yes, the amalgam is overhanging, I will fix that later)

I bored a couple of small holes in the amalgam on the occlusal of #30 and the cervical of #29 and had enough clearance on #27 to bond to the enamel.  I bent a paperclip with ortho pliers and stuck it #30 and bonded to #27 with Luxacore.  For #28, the tooth we want extruded, I put a wire in the cervical hole of #28 and made a little ball of resin on the sharp end (pics 2 and 3).  Check clearance and there were a few mm's for the tooth to move (not shown).

A week later, he comes in and the tooth has clearly moved (compare pic 3 to pic 4).  The P cusp of #4 was hitting the DL amalgam (pic 6).  We reduce #28 my a couple more mm's and checked clearance in CO (pic 5 is after reduction…sorry pic 5 and six are reversed in sequence).

I'll see him again in a week.
